Annual Physical Health Check
If you 18 years and over and have a diagnosis of severe mental health problems such as schizophrenia, bipolar disorder or psychosis then you may need to take extra care with your physical health. Having a health check means that any physical problems can be identified early and suitable interventions offered promptly.
We can help you with this by carrying out an annual health check, please book your appointment with our nurse when invited. It will comprise of a blood test and physical review. Your GP will then contact you afterwards to complete your review the results, medications and treatment plan.
NHS Health Checks Age 40-74 Years Of Age
The NHS Health Check is a health check-up every 5 years for adults in England aged 40 to 74 who do not already have a pre-existing condition such as diabetes High blood pressure etc. It’s designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia.

Annual Health Check For Those With A Pre-existing Medical Condition
All those with a pre existing medical conditions such as diabetes hypertension, heart disease, respiratory disease( asthma COPD), kidney disease, hypothyroidism and dementia will be called for an annual health check and medication review.
