Doctor’s Sick / Fit Note for more than 7 days

If you have been ill for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Fitness To Work’ Certificates.

The doctor may call you or arrange for an appointment.

If you have already been seen by your Doctor about this illness your certificate will be sent to you within a week.

Please read the following:

  • Sick / Fit notes for the first 7 days of illness – please complete the Self Certification Sick / Fit note form that can be downloaded from the practice website.
  • Fit/Sick notes for illnesses after 7 days can only be provided if you have had contact with a health professional during that time.
  • We are unable to provide to provide certificates of return to work. You should go back to work as soon as you feel able to and with your employer's agreement - this may be before your fit note runs out. You do not need to go back to see your healthcare professional before going back to work. Your healthcare professional cannot give you a fit note stating that you are 'fit for work'.

If you have been off work more than 7 days due to illness and had a consultation with a health professional during that time, please proceed with this form.
